WHO IS ELIGIBLE?
Any individual, group, business or organization whose volunteer activities in North Carolina benefit the community or the Triangle Area of North Carolina in a substantial, important or unique way.
Students receiving course credits are eligible, only if the volunteer service extends beyond course requirements. This must be clearly indicated in the nomination statement.
Persons nominating themselves, spouses or family members
Nominations are accepted from January 1 to July 31.
WHO IS NOT ELIGIBLE?
Individuals involved in “court mandated community service” or those serving as “loaned executives”
Persons who receive some compensation for services rendered but there works is exceptional or above and beyond the call of duty to aid people with disabilities. Call or write if you have questions.
ALL SECTIONS OF THE FORM MUST BE COMPLETED. Incomplete forms will not be accepted by the DAC Award
All nominations must be submitted on the official DAC Award nomination form. Photocopies of the form are acceptable.
The nomination form must be printed or typewritten.
All entries must show positive representations of people with disabilities in situations or activities that reinforce such areas as empowerment, inclusion, mainstreaming, accommodating, increasing public awareness, removing economic/social barriers, enhancing the quality of life, demonstrating acts of generosity/kindness, etc.
